Use the ID card guides and samples below to help you identify eligible Select Health members and their network. Member ID cards vary from state to state and by plan type.
Our Member Services representatives can answer your questions about benefits and eligibility. Call us at 800-538-5038, Monday-Friday from 7:00 a.m. to 8:00 p.m., and Saturdays from 9:00 a.m. to 2:00 p.m. to verify benefits.
Select Health contracted Providers can request access to our secure Provider Benefit Tool (PBT) where they can also access member benefits, eligibility and more online.
