Traditional Plans
These plans offer comprehensive coverage with a variety of deductible options. Some plans even have flat copays for office visits, so you don’t have to worry about meeting your deductible first.
Qualified High Deductible Plans (QHDHP)
These plans generally have lower premiums, but higher out-of-pocket costs. However, these plans can be appealing because they can be paired with a tax-advantaged Health Savings Account (HSA).
Copay Plans
Select Health offers copay plan options with predictable costs and easy-to-understand benefit designs. If you like simple, these are the plans for you.
Prescription Drug Coverage
We work with local doctors to develop an evidence-based list of covered drugs, and we encourage lower-cost or generic drug use, when appropriate, to provide more sustainable drug costs. Small Employer plans use our evidence-based drug list, RxCore. It covers a wide range of medications and places minimal restrictions on drug selection.
Optional Benefits
Eyewear
Good vision is an important part of overall health. Select Health has partnered with EyeMed Vision Care to offer funded benefits for vision hardware. Eyewear Plans include coverage for contacts, frames, lens and lens treatments, and more. Visit EyeMed and select the Access network to search a complete listing of participating providers and retail locations.
Dental
Small Employers can add dental benefits to their medical plans or purchase standalone dental coverage. We offer several different plan options to fit your employees needs.
Worksite Wellness Tools
Our goal is to help your employees live the healthiest lives possible. We offer many different programs to help your employees stay active. Some of these include health fairs, Wellness Week, Catch a Flight, and other turnkey programs.

Intermountain Employee Assistance Program (EAP)
Help your employees effectively manage stress and be more present. Intermountain EAP includes benefits such as confidential counseling, legal assistance, and a 24/7 crisis hotline.
Utah
Select Health Value®
Select Health Value is our most affordable network and includes all Intermountain Health doctors, facilities (including Primary Children’s Hospital), clinics, and InstacareSM/KidsCareSM locations—that’s 37 hospitals, 600 clinics, and more than 13,500 providers, including specialists you can see without a referral. This network also includes access to dozens of clinics and providers who are aligned with Intermountain Health.
Select Health Value is a great option for members living in Box Elder, Cache, Davis, Iron, Morgan, Salt Lake, Summit, Tooele, Utah, Wasatch, Washington, and Weber Counties. Please refer to your employer for different plan options.
Select Health Med®
The Select Health Med network is available statewide and is a good option no matter where you live or work in Utah. It includes all Intermountain Health facilities, clinics, doctors, and key specialty facilities such as the Huntsman Cancer Institute and Moran Eye Center. Select Health Med includes nearly 42 in-network hospitals and over 600 clinics with more than 14,000 providers, including specialists throughout the state.
Idaho
In Southwest Idaho, our network provides access to St. Luke’s (SLHP) and Saint Alphonsus (SAHA) hospitals, facilities, and providers, while our network in Eastern and Northern Idaho provides access to Select Health Med and BrightPath providers and facilities. No matter where you are in Idaho, Select Health has you covered with a large network of providers and facilities.
*Small Employer plans are not currently available for purchase in the following counties: Bannock, Benewah, Bonner, Boundary, Butte, Caribou, Clark, Clearwater, Idaho, Latah, Lewis, Nez Perce, Power, and Shoshone.
Nevada
Select Health Value® (HMO)
The Select Health Value® network is integrated with Intermountain Health and provides access to providers and facilities throughout Clark and Nye Counties. With Select Health, you can count on comprehensive coverage with a large network of providers and facilities. Referrals are required to see certain specialists. This network also includes access to dozens of clinics and providers who are aligned with Intermountain Health.
Select Health Med® (POS)
Select Health Med includes all Intermountain Health facilities, clinics, and doctors and key specialty facilities. Select Health Med contracts with many specialists, and no referral is required to see one. Select Health Med provides access to providers and facilities throughout Clark and Nye Counties in Nevada and all of Utah and Idaho.
Additionally, our Select Health Med plans include in-network access across the U.S. through the UnitedHealthcare Options PPO network (for urgent and emergent care).
