Costs vary based on whether a drug is generic or brand-name, its formulary tier, and whether it is part of your plan’s preferred list.

If your medication isn’t covered, your doctor may request a coverage exception or recommend alternative medications that are included in your plan.

Some medications require prior authorization, meaning your doctor must confirm medical necessity and determine other requirements before your plan will cover the prescription.

Most plans have preferred pharmacy networks. Using in-network or preferred pharmacies can help lower your out-of-pocket costs. Use the pharmacy locator tool to find in-network pharmacies near you.
