Thank you for being part of Select Health and for the service you provide to our members. Select Health is a not-for-profit health plan serving more than 900,000 members in Utah, Idaho, and Nevada. Select Health networks include commercial, Medicare Advantage, Medicaid, and CHIP plans.

Please be sure to:

• Verify the networks on which you participate. You can find that information in the appendices attached at the end your Participating Provider Services Agreement (PPSA).
• Use your clients’ Member ID cards to identify plans and verify benefits. Contact Member Services at 800-538-5038 for any questions on member eligibility.
